Here is a picture so you can create your own. I have had three cases where the negligence has been on the trainer/staff of said gym because of lack of this report.
1- The indecent is brought to your attention…present of not.
2- Call 911 or get emergency first aid kit.
3- Tell the personal injured “I am here to assist and make sure your will be OK” Do not ask “Are you OK? Really?
4-Clear the area give this person breathing room..no one is allowed near this person other than staff members, and medical professionals.
5- Tape, bandage, ice, and pamper until medical professional arrive. Don’t talk, or ask too many questions…just say I am here for you, and help is on the way.
6- If they insist they need no help…they need to literally write “I am OK, and I release all parties employed, contracted, by xyz fitness center. I refuse help, and I am leaving at my own free will.
